CFC Services

Community First Choice (CFC) is a Medicaid Program offered to individuals enrolled in Home

and Community-Based Services (HCBS). CFC was implemented on July 1, 2025, with the mission

expanding long-term care services to those enrolled in Health First Choice (Medicaid). Overall,

CFC offers individuals in services greater choice and control over how they receive services.

The goal of Personal Care is to assist the individual in services complete their daily tasks and their activities of daily living. Services include: 

• Acquisition of Skills
• Eating/Feeding
• Personal Hygiene: 

              Bathing 

              Toileting 

              Grooming 

              Shaving 

              Hair Care

              Nail Care 

              Mouth Care 

              Dressing

              Walking 

              Exercise 

              Transferring 

              Protective Oversight 

The goal of Personal Care is to assist the individual in services complete their daily tasks and their activities of daily living. Services include:

• Acquisition of Skills
• Appointment Management
• Banking/Money Management
• Bed Making
• Dishwashing
• Laundry
• Meal Preparation
• Routine Light Housecleaning:  dusting, vacuuming, mopping, and the cleaning of bathroom and kitchen areas
• Shopping

• July 1, 2025: CFC becomes active for eligible members.
• July 2025-2026: Individuals enrolled in HCBS services will begin transitioning to CFC during their Service Plan Meetings. For new members enrolling in services, CFC will be available. 
• Fall 2026-Janaury 2027: CFC services, such as Personal Care and Homemaker, will be removed from HCBS waiver and will be offered exclusively under CFC. 

• Personal Care will be exclusively offered under CFC.
• Homemaker will be exclusively offered under CFC. 
• Homemaker Enhanced service will be discontinued by June 30, 2026. 
• Extraordinary Cleaning (HCBS) service is introduced on July 1, 2025.
